1. Linking to something that changes, or disappears
This is the big one. The code itself doesn't break. The destination does. Once that happens, every printed copy out there becomes dead weight.
Fix: point the code at a URL you control and can update forever, a redirect you own, not the raw address of a page that might not exist next year.
2. Making it too small, or too shiny
QR codes need contrast and a bit of breathing room to scan reliably. Print one at postage-stamp size on a busy background, or emboss it on brushed metal, and you're asking a phone camera to do the impossible in bad lighting. Restaurants are especially guilty here, a code etched into a dark tabletop under mood lighting is basically unreadable.
Fix: keep a clean quiet zone around the code, use dark-on-light contrast, and test it printed at the actual size it will be used, not zoomed in on a screen.
3. No fallback for people who can't or won't scan
Not everyone wants to open their camera app in public, and not every phone cooperates on the first try. If the QR code is the only way to get the information, say, a menu or an event schedule, you've locked out a chunk of your audience for no reason.
Fix: always give people an alternative. A short printed URL, a phone number, a name they can ask a staff member for. The code should be a shortcut, not a gate.
4. Sending people to a page that isn't mobile friendly
